Medication: Please come to the health room on or before the last day of school, May 22, to pick up your student’s medication. An adult must pick up the medication; we cannot send it home with your student unless they are self-carry authorized (inhalers only). Any medications left after the last day of school will be discarded.

Forms: New ‘Permission to Administer Medication’ forms are needed for each school year and can be picked up at the school health office and found on the ASD20 website under medical forms and documents within parent resources.

Care Plans: If your student has an emergency action plan and/or emergency medications at school (i.e. Allergies & Anaphylaxis, Asthma, Seizures) new care plans are needed each school year and can be picked up at the health office and found on the ASD20 website under medical forms and documents within parent resources. If your student has an individual health care plan, this form will be sent directly to you.

Non-Medical Immunization Exemptions: Non-medical immunization exemptions are required to be completed annually no sooner than July 1 for the upcoming school year. Exemption forms are found on the Colorado Department of Public Health and Environment website.

Picking up the forms and health care plans in May will allow you time over the summer to obtain the appropriate signatures. If your child had medications and/or health care plans at school this year, copies of these forms will be emailed to you for your convenience, but you can access them anytime from the websites provided above.
